How Legionnaires' Disease Is Treated in Older Adults
When an older parent is hospitalized with Legionnaires' disease, every hour of delay increases the risk of ICU admission and death. This guide explains the treatment timeline, diagnostic blind spots, what to ask the medical team, and what recovery realistically looks like so you can advocate effectively.
In the first day after Legionnaires' disease enters the conversation, the most important question is not whether every test result is back. It is whether the hospital has already started an antibiotic that actually treats Legionella.
That is the hard clock behind treating Legionnaires' disease in older adults. In a Mayo Clinic single-center review reported by CIDRAP, patients who received appropriate therapy within 24 hours had a lower ICU admission rate than those whose treatment was delayed: 54.2% versus 80.4%.[1] The study cannot promise what will happen to one parent in one hospital bed, but it explains why a family member should not be brushed off with, "We're waiting on the urine test," if the clinical team is already worried about Legionella.

A useful bedside question is simple: "Has my parent already received an antibiotic that covers Legionella, and when was the first dose given?" Write down the answer, including the medication name and time. That note may matter later when the plan changes, a new consultant comes in, or the oxygen numbers start moving in the wrong direction.
The First 24 Hours Are About Treatment, Not Certainty
Legionnaires' disease is a form of pneumonia caused by Legionella bacteria. In an older adult, it can look at first like other serious pneumonias: fever, cough, shortness of breath, weakness, confusion, low oxygen levels, diarrhea, or a general decline that seems faster than the family expected. The hospital may start with the broader label of "pneumonia" while it sends tests to identify the cause.
That uncertainty is normal. Waiting to treat is not always safe. A 2022 clinical review reported that the median time from symptom onset to appropriate antibiotics was 5 days among non-survivors and 1 day among survivors.[2] That does not mean a caregiver can calculate a parent's outcome from a calendar. It does mean the treatment timeline deserves a direct conversation.
The first conversation with the team should stay practical:
- What pneumonia antibiotics has my parent received so far?
- Do those antibiotics cover Legionella?
- When was the first dose that covered Legionella?
- If the first Legionella test is negative, will you still keep Legionella on the list?
- What changes in breathing, blood pressure, kidney function, or confusion would make ICU care necessary?
Those questions are not an attempt to manage the case from the visitor chair. They are a way to make sure the treatment clock is visible to everyone in the room.
Why a Negative Urine Test May Not End the Search
The diagnostic trap with Legionnaires' disease is that one test can sound more complete than it is. Families often hear "urinary antigen test" and reasonably assume it means, "This will tell us whether it is Legionella." It may, but it may not tell the whole story.

Urinary antigen testing, often shortened to UAT, looks for Legionella antigen in urine. It is fast and useful, especially for the most common Legionella pneumophila serogroup. But it is not the same as PCR, which detects Legionella genetic material from a respiratory sample, and it is not the same as culture, which attempts to grow the organism from a specimen.
That distinction is not academic. In the Mayo Clinic review reported by CIDRAP, urinary antigen testing alone missed 74.4% of PCR-confirmed Legionella cases.[1] The review was retrospective and from a single center, so it should not be treated as a universal miss rate for every hospital. Still, it is strong enough to change the bedside question from "Was the urine test negative?" to "If suspicion remains, are PCR or culture being sent too?"

The careful wording matters. A caregiver should not demand a specific test as if every hospital situation is identical. But if an older parent is worsening, has pneumonia that is not behaving as expected, or has risk factors that make Legionella plausible, it is reasonable to ask whether the diagnostic plan goes beyond UAT.
Which Antibiotics Are Commonly Used
Legionella does not respond to every antibiotic used for ordinary bacterial pneumonia. Authoritative clinical references identify azithromycin and levofloxacin as first-line options for Legionnaires' disease.[2][3] In the hospital, treatment is often given intravenously at first, especially when an older adult is very ill, cannot reliably take pills, has vomiting or confusion, or needs close monitoring.
The exact choice depends on kidney function, heart rhythm risk, medication interactions, allergy history, illness severity, local practice, and whether other infections are also being covered. This is where a medication list becomes more than paperwork. If your parent takes drugs for rhythm problems, blood thinning, seizures, transplant care, diabetes, or psychiatric conditions, make sure the team has the current list rather than an old one copied forward from the chart.
Typical treatment durations are often described as 7 to 10 days for moderate illness, with courses up to 21 days in severe disease or in immunocompromised patients.[2][3] Those ranges are not a prescription for your parent; they are a reason to ask, before discharge, "How many total days of Legionella-active antibiotics are planned, including the doses already given in the hospital?"
One more practical point: when the hospital changes from IV antibiotics to pills, that does not necessarily mean the infection is gone. It may mean your parent is stable enough to absorb oral medication and continue treatment outside the hospital. Ask what would count as failure of the oral plan: fever returning, worsening shortness of breath, inability to keep medication down, new confusion, or oxygen needs rising again.
Why Older Adults Can Deteriorate Quickly
Age by itself is not the whole story, but it is part of the risk profile. The World Health Organization states that 75% to 80% of reported Legionnaires' disease cases occur in people age 50 or older.[4] Smoking history, chronic lung disease such as COPD, diabetes, and immunosuppression are also recognized risk factors.[4][5]
Those risks do not mean a parent will do badly. They help explain why the team may watch oxygen levels, breathing effort, blood pressure, kidney function, sodium levels, mental status, and urine output so closely. An older adult may have less reserve; a pneumonia that a younger person tolerates on a regular hospital floor can push a frailer patient toward ICU-level support.
In the CIDRAP-reported Mayo Clinic cohort, 36% of hospitalized patients required ICU care and 23% required mechanical ventilation.[1] Mortality was 4.6% among patients treated on medical wards and 23.1% among those admitted to the ICU.[1] Those numbers are frightening, but they are also clarifying: ICU transfer is not a sign that someone has given up. It may be the place where the team can deliver the level of oxygen, blood pressure support, monitoring, and rapid response the illness now requires.
A broad clinical review reported an overall case-fatality rate around 10%, rising to 25% in healthcare-associated cases and above 40% in immunocompromised older adults.[2] These are population-level figures, not a forecast for one person. But if your parent is immunocompromised, recently hospitalized, in a long-term care setting, or already medically fragile, it is worth asking the team to explain how those factors change the monitoring plan.
What ICU-Level Treatment May Involve
If the team starts discussing ICU transfer, listen for the reason. Is the issue oxygen that cannot be maintained on the current floor? Is breathing effort increasing? Is blood pressure unstable? Is confusion making it hard to protect the airway or cooperate with oxygen support? Is kidney function worsening?
ICU care can include higher levels of oxygen support, noninvasive ventilation, mechanical ventilation, IV fluids, medications to support blood pressure, closer lab monitoring, and more frequent reassessment. The antibiotics still matter, but ICU care is also about buying time for the lungs and other organs while the infection is treated.
For the family, the most useful questions are not abstract. Ask what the team is watching over the next few hours, what would show improvement, and what would trigger the next step. If mechanical ventilation is mentioned, ask who will call you, how quickly decisions may need to be made, and whether the hospital has your parent's advance directive or health care proxy documentation.
This is also the moment to tell the team what your parent was like before the illness. Could they walk to the bathroom alone? Did they use oxygen at home? Were they already confused, or is this new? Did they manage their own medications? Baseline function helps clinicians judge whether a change is infection-related, medication-related, delirium, or part of a longer decline.
The Bedside Notebook Questions That Matter Most
In a fast-moving hospitalization, the same family member often hears different words from different people: pneumonia, atypical pneumonia, Legionella, urinary antigen, PCR, culture, azithromycin, levofloxacin, oxygen requirement. The notebook is not for catching anyone in a mistake. It is for keeping the thread intact across shift changes.
- Diagnosis: What tests have been sent for Legionella, and which are still pending?
- Treatment timing: When did the first Legionella-active antibiotic start?
- Medication plan: Is the antibiotic azithromycin, levofloxacin, or another option, and why was it chosen?
- Severity: What is the current oxygen support, and is it increasing, stable, or decreasing?
- Escalation: What specific changes would lead to ICU transfer?
- Discharge: How many antibiotic days remain, and what symptoms should prompt an urgent call or return to the hospital?
One question is especially easy to miss: "If the urinary antigen test is negative, does that change the plan, or are we still treating while PCR or culture is pending?" The answer may depend on how sick your parent is and how strongly the clinical picture points toward Legionella. But asking makes the diagnostic blind spot visible.
Discharge Is a Handoff, Not the End of Treatment
Families often feel enormous relief when discharge appears on the board. Then the instructions arrive: finish antibiotics, follow up with primary care or pulmonology, use oxygen if prescribed, watch for fever, arrange therapy, prevent falls, come back if breathing worsens. The relief is real. So is the work.
Before leaving, ask the team to say the home plan out loud. How many antibiotic doses are left? What time should the next dose be taken? Are there food, antacid, supplement, or medication interactions? Who is responsible for follow-up imaging or lab work? If oxygen is prescribed, who delivers it, what flow rate should be used, and what oxygen saturation range should trigger a call?
Older adults may also leave the hospital weaker than they entered it. That weakness has consequences: bathroom trips take longer, stairs become risky, appetite lags, sleep is broken, and a person who was independent with pills may suddenly need help. A discharge plan that assumes full baseline function can fail even when the antibiotic choice is correct.
If your parent lives alone, ask directly whether they are safe to return there now. If the answer is yes, clarify what support is needed for the first week: medication setup, meals, hydration, transportation, bathing, oxygen equipment, and someone checking for confusion or worsening breathlessness. If the answer is no, ask whether home health, rehabilitation, or a short-term skilled nursing stay is being considered.
Recovery Can Take Months
Surviving Legionnaires' disease does not always mean returning quickly to the person who walked into the hospital. Fatigue can be stubborn. Thinking can feel slower. Muscles may ache or feel unreliable. Families sometimes interpret this as lack of effort, depression, or normal aging, when it may be part of recovery from a severe infection and hospitalization.
One often cited follow-up study of 122 survivors from a single Dutch outbreak found that, more than 17 months later, 75% reported fatigue, 66% reported neurologic symptoms, and 63% reported neuromuscular symptoms.[6] Because the data came from one outbreak cohort, it should not be treated as a universal prediction. It does, however, give language to what many families notice after discharge: the body may be technically past the infection but still far from recovered.
Patient education sources commonly describe most people as recovering fully within a year, though recovery speed varies with severity, age, immune status, lung disease, ICU stay, and pre-illness function.[7] For an older parent, a realistic recovery plan may need primary care follow-up, pulmonary follow-up, medication review, physical therapy, nutrition support, sleep repair, and a slower return to errands, driving, and caregiving responsibilities.
The follow-up questions change after discharge, but they are still concrete:
- Is breathlessness improving week by week, staying flat, or worsening?
- Is fatigue severe enough to interfere with eating, bathing, walking, or taking medication?
- Is confusion new, improving, or becoming a safety problem?
- Has the antibiotic course been completed exactly as prescribed?
- Does the follow-up clinician know there was suspected or confirmed Legionnaires' disease, not just "pneumonia"?
Caregivers cannot diagnose Legionnaires' disease from the bedside, choose the antibiotic, or control whether a frail parent needs ICU care. But they can keep the right questions in the room early enough to matter: antibiotic timing, testing beyond urinary antigen when suspicion remains, signs of respiratory decline, total treatment duration, and a recovery plan that matches the person coming home.
